Mental Capacity
The signer must be of sound mind and able to understand what they are signing at the time of notarization. If there are concerns about mental capacity, we may recommend consulting with the attending physician or an attorney.
Valid Identification
The signer must have a valid, government-issued photo ID. If the patient does not have their ID at the hospital, a credible witness who knows the signer may be used in some circumstances.
Witnesses
Some documents, such as wills and healthcare proxies, require witnesses in addition to the notary. Please arrange for appropriate witnesses to be present. We can advise on witness requirements when you call.
